NTIAC fees for the review of medical records and / or videos:

    

Review of video with or without medical information:  $200

    

Review of medical information with or without photos:  $150

          **Fees include a phone follow up to discuss results
       & further phone or e-mail contacts prior to the adoption.
        

Personal office consultation with Dr. Eckel:  $150

    **This fee is in addition to the above review fees. These personal                 consultations are extensive and take and extra amount of time.          It is a chance to discuss the records, preparation for the adoption                                              trip and adoption issues in general.

Please send check or money order payable to Bruce Eckel, MD, include
the information to be reviewed, AND the Waiver of Liability and Personal Data Sheet.

      Please note that a review will not be done unless all of the above are received.
